TERMS AND CONDITIONS
1. GENERAL PROVISIONS
Read and accept the “General Terms and Conditions” defined and described by the organizers of the Convention in order to register as a participant for the 9th Cornelia de Lange Syndrome International Convention. If you do not agree and accept the terms and/or conditions set forth in this document, you will not be able to register for the 9th Cornelia de Lange Syndrome Convention. If you have any questions related to these General Terms and Conditions, please contact the organizers of the Convention by sending an e-mail to: email@example.com.
2. . REGISTRATION FORM
Registrations for the Convention will be carried out through a form available on the ‘REGISTRATION’ page. Username and Password must be provided to access the form. The registration will be provided per family, being necessary to start with the information of the person with CDLS in order to complete the “REGISTRATION FORM”.
3. PARTICIPANT INFORMATION
The information of participants of the Convention will be available in the mailing system of CDLS Brazil for assistance during the Convention and for future contact.
4. PERMISSION FOR THE USE OF IMAGES
The authorization for the subsequent propagation of photos and images of participants in the Convention must be provided on the “REGISTRATION FORM” by the person registering.
5. MEDICAL SPECIALTIES
During the Convention, there will be individual consultations with several medical specialists. During the registration, each family will be able to choose 5 (five) medical specialties, in their preferred order. However, in this option, 3 (three) specialties will be available for assistance, according to the availability. The bearer does not have to be present for the consultations, thus, it is left to the discretion of each family.
The food available at the hotel during the Convention will be the same for all participants. The parents or caretakers of the bearer will be responsible for providing specific meals. The Convention will not offer specific meals related to the diet of each person with CDLS.
Caretakers, parents and caregivers will be responsible for handling the medications of the person with CDLS. The organizers of the Convention will not provide any type of medication.
The family of the bearer is responsible for providing a caregiver to accompany him/her during the Convention. There will be an area for support on the site, however, the directors of the Convention will not be responsible for individual monitoring.
9. AMBULANCE AND HOSPITAL ASSISTANCE
There will be a Mobile ICU ambulance available on the site for basic care to assist all participants of the Convention. If hospital care and hospitalization is needed, the Convention Organizer will provide the initial support necessary, however, the families will be responsible for any hospital arrangements and costs, as well as costs with special transportation.
Insurance: It is recommended for families to get insurance for their specific needs, thus protect themselves from any unforeseen circumstances.
Accommodations will include single, double, triple or quadruple rooms, according to the number of participants of each family registered. The price for the accommodation chosen by the family shall be included in the price of the registration, totaling the amount due in the final confirmation of the registration.
Tauá Resort Caeté, location of the Convention, is located 76 Km from Confins Airport. On August 15, the Convention Organizer will transfer “groups” from Confins Airport to Tauá Resort Caeté at pre-scheduled times. On the registration form, each family must provide information regarding their flights in order for the Convention Organizer to elaborate the transfer schedule. The schedule of the transfer offered will be posted on the website and can be checked when the date of the Convention is approaching. There will be a special waiting area at Confins Airport for the families in case they have to wait for the next transfer time.
Note: The family may choose a private transfer. In this case, the family will be responsible for the hiring and payment. For more convenience, this information will be made available on the website.
The payment for registrations and accommodations must be made by credit card, after the REGISTRATION FORM is completed. A payment via bank slip will also be available for the registration of Brazilian families.
13. CANCELLATION OF PARTICIPATION
To cancel a registration, contact the Convention Organizer by sending an email to firstname.lastname@example.org.
Cancellation requests will be reviewed by the Organizer. Note: Tauá Resort Caeté will not provide a refund of the payment after the accommodation is confirmed, in accordance with clauses defined in the contract.
